Nominate

• The Florida Civil Rights Hall of Fame is open to all persons, regardless of race, color, religion, sex, national origin, age, handicap/disability, or marital or familial status (pursuant to Chapter 760, Florida Statutes).
• Current Commissioners and staff of the Florida Commission on Human Relations and their immediate families are not eligible to be inducted into the Hall of Fame.
• Persons, living or deceased, who were either born in Florida or adopted Florida as their home state and base of operation, are eligible to be nominated.
• Self-nominations are acceptable.
• Only nominations submitted on the official nomination form will be accepted.
• Incomplete nominations will not be forwarded to the Selection Panel for consideration.
• Electronic submissions are preferred; however, hardcopy nomination packets will be accepted.
• The nomination form and accompanying materials will become the property of the Florida Commission on Human Relations as the state entity responsible for administering the Florida Civil Rights Hall of Fame. All information submitted to the Commission will be subject to Florida’s public records law (Chapter 119, Florida Statutes).
• Any materials included in the nomination packets will NOT be returned to the nominee or nominator.

The submitting party agrees to indemnify, defend and hold the Florida Commission on Human Relations harmless against and from any claims of liability, suits or damages, including attorney’s fees and costs, arising from the Commission’s use of the information submitted by or on behalf of any nominee or related to any failure to obtain necessary licenses or consents and/or for infringements or other violations of the patent, copyright or other intellectual property rights, or the rights to privacy of publicity of any third party. The nominees, their representatives and any person selected further authorize the reproduction, adaptation, public distribution and public display of the submitted materials in electronic or printed media, television or similar means.

Individual must:

• Have been born in Florida or adopted Florida as his or her home state and base of operation;
• Beat least 18 years of age; and
• Have made a significant contribution and provided exemplary leadership toward Florida’s progress and achievements in civil rights. (Contributions can include community service and empowerment, legislative advocacy, grassroots organizing, coalition building, authored publications, organizational leadership, private and public diversity efforts and multicultural educational initiatives in any area of civil rights or related areas.)

When choosing someone to honor as a potential candidate into the Florida Civil Rights Hall of Fame, consider the following questions:

• Is there a higher quality of life for people living in Florida or its communities due to the nominee’s contributions?
• Has there been a learning experience afforded to Floridians due to the nominee’s contributions?
• Has the status of people in Florida been improved or have opportunities been enhanced because of the nominee’s contributions?
• Has Florida or its communities, the nation, the world or a particular discipline, profession or field used the contributions of the nominee as a model of its own?

Nominations can be submitted in the electronic form found on this page or in print by downloading the nomination package and filling it out for mailed submission to the Florida Commission on Human Relations listed below.

The completed nomination form should be no more than ten (10) typed or handwritten pages in length. Compact disks (CDs) are acceptable. Do not send audio or videotapes; however, you can refer to a website (provide the complete web address) that has audio/visual components. Due to reproduction quality, we cannot accept faxed submissions.
